Our relationship with the party that brings you Achieve Solutions

The Achieve Solutions site is owned and operated solely by Carelon Behavioral Health, an affiliate of Carelon Behavioral Health of California, Inc. Carelon Behavioral Health may partner with third parties, including employers, government agencies and health plans, in providing the services found on this site. While our partners may receive aggregate information about site use by their employees or members, any personal information that you disclose to us on this site will be retained on the server of Achieve Solutions and Carelon Behavioral Health, and Carelon Behavioral Health will not share that information with any third parties, except as provided in this Privacy Statement.

Your IP address

For each visitor to the site, Achieve Solutions servers automatically collect information about which pages are visited, as well as the IP address and domain name (e.g., achievesolutions.net) of visitors. We use your IP address to help diagnose problems with our server. For example, your IP address could be used to determine your general location at the time you log in, and this information might be useful in diagnosing the problem you are experiencing.

Cookies

Limited Disclosure of Information

We share certain aggregated data regarding the use of this site with employers, plan sponsors and Carelon Behavioral Health of California, Inc. Carelon Behavioral Health will not provide your employer or plan sponsor with any information regarding your personal use of this site, but your employer or plan sponsor may receive reports showing aggregate use of the site.

Achieve Solutions does provide Carelon Behavioral Health and Carelon Behavioral Health of California with a user’s personal identifying information (name and telephone number) when the user completes the “Callback Request” form requesting a callback from an EAP professional. The Achieve Solutions server will retain eligibility data, information which is entered on the Callback Request Web page and anonymous user demographic information.

Carelon Behavioral Health, the parent company of Carelon Behavioral Health of California, provides computer backup services of the information retained on Achieve Solutions’ servers. Carelon Behavioral Health also maintains the email server that is used in conjunction with the callback request form and other email communications connected to email addresses displayed on the Achieve Solutions website.

Carelon Behavioral Health may disclose individual account information in special cases (i) to comply with valid legal requirements, such as a law, regulation, search warrant, subpoena or court order; or (ii) when Carelon Behavioral Health has reason to believe that disclosing this information is necessary to identify, contact or bring legal action against someone who, intentionally or unintentionally, may be causing injury or harm to Achieve Solutions users or other persons. For example, Carelon Behavioral Health may disclose information necessary to comply with laws governing the reporting of child abuse or abuse of disabled adults.

Security

This site has security measures in place to protect against the loss, misuse and alteration of the information under our control. Achieve Solutions is a VeriSign Secure Site, which enables visitors to verify the authenticity of the site.
