Achieve Solutions does provide Carelon Behavioral Health and Carelon Behavioral Health of California with a user’s personal identifying information (name and telephone number) when the user completes the “Callback Request” form requesting a callback from an EAP professional. The Achieve Solutions server will retain eligibility data, information which is entered on the Callback Request Web page and anonymous user demographic information.
Carelon Behavioral Health, the parent company of Carelon Behavioral Health of California, provides computer backup services of the information retained on Achieve Solutions’ servers. Carelon Behavioral Health also maintains the email server that is used in conjunction with the callback request form and other email communications connected to email addresses displayed on the Achieve Solutions website.
Carelon Behavioral Health may disclose individual account information in special cases (i) to comply with valid legal requirements, such as a law, regulation, search warrant, subpoena or court order; or (ii) when Carelon Behavioral Health has reason to believe that disclosing this information is necessary to identify, contact or bring legal action against someone who, intentionally or unintentionally, may be causing injury or harm to Achieve Solutions users or other persons.
